All Saints Hospice has a Personal Touch

All Saints Hospice provides an individualized personal care plan to fit your individual needs. Our personal care is provided by nurses and nurses aides and includes services such as bathing, feeding and maintaining daily personal hygiene. Additionally, if there are other specialized care demands we will incorporate these into your daily personal care plan.

Indicators that are hospice-eligible can include but are not limited to:

Weight Loss
Increased assistance with daily activities
Increased weakness and fatigue
Frequent and multiple falls
Difficulty swallowing
Shortness of breath or chest pain

Continuous oxygen
Increase/changes in medication
Recurring or multiple infections
Frequent phone calls to physician
Frequent hospitalizations or ER visits

Everything You Need

If you or your family should ever need anything in addition to our scheduled care, our staff is on-call 24 hours a day, 7 days a week. Our hospice care team provides everything from registered hospice nurses, experienced aides, financial and spiritual counseling, and any necessary medical equipment. Your care plan is designed so you can live comfortably surrounded by your loved ones.
